New Patients and How to Register at Alexander House Surgery.

The practice is accepting new patient registrations.  Please see map below to see whether you are in our catchment area.   

If you live within our catchment area and you wish to register here, then call into the surgery to complete the registration document.

You will be asked to complete your details including your address and fill in a medical history questionnaire. Please make sure you have two forms of photographic ID and proof of address for the receptionist to photocopy.  

You will also be invited to make an appointment to attend a new patient medical. 

On completion of these forms you will be registered at the practice and be able to make an appointment to see a doctor. We would request that you keep us informed of any changes to your personal details whilst registered at the surgery, thank you. Below is an electronic copy of the new patient registration form and new patients questionnaire which both need to completed when registering at the practice.  You may wish to print these, fill them out and bring them in with you on your first visit to the practice to speed up the registration process.

Practice Boundary

Patients within the line are welcome to register at Alexander House Surgery. Due to large demand we will not accept any patients outside our practice boundary. Please use the NHS website to find alternative surgeries.
