What is Patient Access?
GP online services allow you to access a range of services via your computer, mobile or tablet. Once you have signed up, you will be able to:
• Book or cancel appointments online
• Renew or order repeat prescriptions online
• View parts of your GP record, including information about medication, allergies, vaccinations, previous illnesses and test results
You can still contact the practice by phone or in person, this is just another option, which other patients have found is more convenient and saves them time. Alternatively you can register for online access via ‘Patient Access’. You will need to fill in a short form at our reception desk and bring proof of your identity – 2 forms of documentation must be provided, one of which must contain a photograph. Acceptable documents include passports, photo driving licences and bank statements. It is important that before you apply you are confident that nobody else in your household will be able to access your medical record against your wishes. You may give your consent to someone else to have proxy access, for example relatives or carers, by requesting a Proxy Access to Medical Records registration form at our reception desk. Proxy access allows parents, family members, carers, care home staff, to have access to their patients’ online records, in order to book appointments, request repeat medication and where applicable, view their medical record. The patient could be 9 or 90 and in need of some assistance in managing their healthcare. * If a patient under the age of 16 has a proxy, access is restricted at 11 and stopped at 16.
